Use the fields and options to configure the Employee Personal Info Screen.

Employee Personal Information

Field Description
Last Name

This non-editable field displays the last name of the employee referenced in this transaction.

First Name

This non-editable field displays the first name of the employee referenced in this transaction

Middle Name

This non-editable field displays the middle name of the employee referenced in this transaction.

Preferred Name

This non-editable field displays the preferred first name or "nickname" for the employee referenced in this transaction.

Suffix

This non-editable field displays the suffix, such as Junior or III, for the employee referenced in this transaction.

Social Security Number

This non-editable field displays the social security number of the employee referenced in this transaction.

Birth Date

This non-editable field displays the date of birth for the employee referenced in this transaction.

Gender

This non-editable field displays the gender of the employee referenced in this transaction.

Race

This non-editable field displays the race of the employee referenced in this transaction.

Employment Information

Field Description
Title This field displays the employee's position title.
Hire Date This non-editable field displays the hiring date for the employee referenced in this transaction.
Service Date If the employee leaves the company and later returns, this field displays the earlier hire date (the previous date of hire).
Last Review Date This field displays the date of the employee's last performance review. This field is for informational purposes only.
Next Review Date This field displays the date of the employee's next performance review. This field is for informational purposes only.

Visa Information

Field Description
VISA Type This field displays the value for the employee's visa type. Visa types are established on the Manage Visa Types screen.
VISA Exp Date This field displays the date on which the employee's visa expires.

Military Status

This group box displays the applicable military statuses for the employee.

Field Description
Disabled Veteran This check box indicates if the employee is a disabled veteran. A disabled veteran is:
  • A veteran of the U.S. military, ground, naval, or air service who is entitled to compensation (or who but for the receipt of military retired pay would be entitled to compensation) under laws administered by the Secretary of Veterans Affairs;
  • A person who was discharged or released from active duty because of a services-connected disability.
Active Duty Warfare or Campaign Badge Veteran This check box indicates if the employee is reported as an Other Protected Veteran in the VETS-4212 report. The U.S. Department of Labor defines “Active Duty Wartime or Campaign Badge Veteran” as a veteran who served on active duty in the U.S. military, ground, naval or air service during a war or in a campaign or expedition for which a campaign badge has been authorized under the laws administered by the Department of Defense.
Special Disabled Veteran This check box indicates if the employee is a Special Disabled Veteran. This data is for informational purposes only, and is not currently used in any Costpoint reporting or processing. Previously, this information was used in VETS-100 report.

The following determines Special Disabled Veteran status:

  • The veteran is a veteran of the U.S. military, ground, naval or air service who is entitled to compensation under laws administered by the Department of Veterans' Affairs for a disability unless he is receiving military retired pay.
  • The disability is rated at 30 percent or more.
  • The disability is rated at 10 or 20 percent in the case of a veteran who has been determined under Section 38 U.S.C. 3106 to have a serious employment handicap.
  • The veteran was discharged or released from active duty because of a service-connected disability.
Vietnam Era Veteran

This check box indicates if the employee is a Vietnam Era Veteran. This data is for informational purposes only, and is not currently used in any Costpoint reporting or processing. Previously, this information was used in VETS-100 report.

The following determines Vietnam Era Veteran status:

  • The veteran served on active duty in the U.S. military, ground, naval, or air service for a period of more than 180 days and was discharged or released with other than a dishonorable discharge, if any part of that active duty was performed:
  • In the Republic of Vietnam between February 28, 1961, and May 7, 1975.
  • Between August 5, 1964, and May 7, 1975, in all other cases.
  • The veteran was discharged or released from active duty in the U.S. military, ground, naval, or air service for a service-connected disability, if any part of that active duty was performed:
  • In the Republic of Vietnam between February 28, 1961, and May 7, 1975.
  • Between August 5, 1964, and May 7, 1975, in any other location.
Protected Veteran (Declined to Self-Identity) This check box indicates if the employee is a veteran who belongs to at least one of the following veterans but has opted not to self-identify a specific category:
  • Disabled Veteran
  • Active Duty Wartime or Campaign Badge Veteran
  • Armed Forces Service Medal Veteran
  • Recently Separated Veteran
Vietnam Era Veteran This check box indicates if the employee is a Vietnam Era Veteran. This data is for informational purposes only, and is not currently used in any Costpoint reporting or processing. Previously, this information was used in VETS-100 report.

The following determines Vietnam Era Veteran status:

  • The veteran served on active duty in the U.S. military, ground, naval, or air service for a period of more than 180 days and was discharged or released with other than a dishonorable discharge, if any part of that active duty was performed:

Armed Forces Service Medal Veteran

This check box indicates if the employee is an Armed Forces Service Medal Veteran. An Armed Forces Service Medal Veteran is a veteran who, while serving on active duty in the U.S. military, ground, naval or air service, participated in a United States military operation for which an Armed Forces service medal was awarded pursuant to Executive Order 12985 (61 FR 1209).

Reservist This check box indicates if the employee is designated as a Reservist. This data is for informational purposes only, and is not currently used in any Costpoint reporting or processing.
Discharge/Release Date If applicable, this field displays the date (in MM/DD/YYYY format) on which this employee was discharged or released from active military duty. This date is used to determine the Recently Separated Veterans to be included in the count of Protected Veterans in the VETS-4212 report.

A Recently Separated Veteran in VETS-4212 report is a veteran during the three-year period beginning on the date of such veteran’s discharge or release from active duty in the U.S. military, ground, naval or air service.
