Contents of the Emergency Contacts Screen
Use the fields and options to configure the Emergency Contacts Screen.
Primary Contact
Use this group box to enter the contact information of the first person to be the contacted during emergencies.

Name |
Enter up to 25 alphanumeric characters for a contact name. Note: This field will be required if you make an entry in the
Relationship or Phone fields.

Relationship |
Enter up to15 alphanumeric characters to describe the relationship to the employee. Note: This field will be required if you make an entry in the
Name or
Phone fields.

Phone |
Enter up to 20 alphanumeric characters for the phone number. Note: This field will be required if you make an entry in the
Name or
Relationship fields.

Phone Formatting |
A display field shows how the phone number should be entered on screen (if it is set to be validated in the Configure Self Service Settings screen). |
Street 1 |
Enter up to 30 alphanumeric characters for a street address, line 1. |
Street 2 |
Enter up to 30 alphanumeric characters for a street address, line 2. |
Street 3 |
Enter up to 30 alphanumeric characters for a street address, line 3. |
City |
Enter up to 25 alphanumeric characters for the city. |
State/Province |
Select the state/province associated with the address entered/displayed on screen. The valid values are the states/provinces set up in Costpoint for the selected country. |
Postal Code |
Enter up to 10 alphanumeric characters for the postal code. |
Postal Code Formatting |
A display field shows how the postal code should be entered on screen (if it is set to be validated in the Configure Self Service Settings screen). |
Country |
Select the country that coincides with the address entered/displayed on screen. The valid values are the countries set up in Costpoint (if you change the Country, the State/Province field will be refreshed). |
Secondary Contact
Use group box to enter the contact information of the secondary person to be contacted during emergencies.
