Brand Name Prescription Amount	This field displays the amount of the brand name prescription copay.
Generic Prescription Amount	This field displays the amount of the generic prescription copay.
Individual Deductible	This field displays the amount of the deductible for the individual.
Family Deductible	This field displays the amount of the deductible for the family.
Payment % After Deductible	This field displays percentage of the amount after the deductible.
Individual Out-of-Pocket Max	This field displays the maximum amount of out-of-pocket expenses for the individual.
Family Out-of-Pocket Max	This field displays the maximum amount of out-of-pocket expenses for the family.
