Benefit Plan Details Tab

Use the Benefit Plan Details tab to define the insurance provider, system/user benefit types, status, and Employee Self Service (ESS) visibility options of the benefit plan.

Field Description
Provider

Enter, or click to select, the code for the provider that is associated with this benefit plan. The provider is the company or agency that provides the insurance policy associated with this benefit plan. Costpoint validates this code against the Provider table, which is set up on the Manage Benefit Providers screen. The name of the provider displays in the field to the right.

System Benefit Type

From the drop-down list, select the type of system benefit. These codes are used by various benefits reports. Valid options are:

Note: If you are setting up a Health Savings Accounts (HSA), select OTHER (Other Benefit) as the System Benefit Type, and then select HSA as the User Benefit Type.
User Benefit Type

Enter, or click to select, the user benefit type code that is associated with this benefit plan. You can establish this codes on the Manage Benefit Types screen. You can use the codes that you defined for internal tracking purposes. These are different from the standard system benefit types. Costpoint validates the benefit type code against the Benefit Types table.

Plan Information URL

Enter the Web location for information about this benefit plan for Employee Self Service exactly as you would type it in a Web browser (for example, "http://www.websitename.com". Employee Self Service is an add-on product for Costpoint that allows employees to view their benefits information online.

Employer-sponsored self-insured health plan

Select this checkbox if the company assumes the financial risk for providing health care benefits to its employees. This is also referred to as a self-funded plan. In practical terms, self-insured employers pay for each out-of-pocket claim as they are incurred instead of paying a fixed premium to an insurance carrier, which is known as a fully-insured plan.

High deductible health plan (HDHP)

Select this checkbox to designate the health plan as a high deductible plan. Health Savings Accounts (HSA) are available only to employees who are enrolled in a high deductible plan.

Employee Self Service

Field Description
Exclude from ESS current elections

Select this checkbox to exclude the benefit plan from ESS Current Elections.

Exclude from ESS benefit options

Select this checkbox to exclude the benefit plan from ESS Benefit Options.

Require HR approval in ESS

Select this checkbox if the plan requires approval before it is effective.

Miscellaneous (Information Only)

Field Description
Core Benefit

Select this checkbox if this benefit is automatically granted to an employee, unless specifically waived.

Eligible for COBRA

Select this checkbox to indicate that this benefit needs to be offered as a part of COBRA continuation. This is for information only.

Coverage Type

Use this drop-down list to define the benefit plan type. This designation is for information only.

  • Point of Service: Select this option if the employee can choose from a network of doctors and hospitals.
  • HMO: Select this option if the employee is required to choose a primary care physician and facility.
  • Indemnity Plan: Select this option if this benefit plan partially reimburses employees for their medical expenses.
  • Other: Select this option if none of the above are applicable (as for a Life Insurance benefit plan). This is the system default.
