State Form 941 Worksheet
The State Form 941 Worksheet provides most of the information (such as, gross pay, 401(k) contributions, 125/Cafeteria contributions, and the state income taxes paid) by state for each employee that your enterprise must file with the your state's tax agency every quarter.
The Schedule B section on the last page of this report lists the daily state tax liabilities for your company, listed by month.
Multiple Companies
If you use multiple companies, this report may contain employee data for one or more companies that are associated with the employee.
