Contents of the Employee Vaccinations Screen

Use the fields and options to configure the Employee Vaccinations screen.

Field Description
Vaccine Type

Enter, or click to select, the type of vaccine. The adjacent field displays the vaccine description.

Vaccination Date

Enter, or click to select, the date when your vaccination occurred.

Dosage From the drop-down list, select the dosage for the selected vaccine. Valid options are:
  • 1 of 1
  • 1 of 2
  • 2 of 2
  • Booster
  • Annual
  • Other
  • Not Applicable
Exempt Select this check box if you are exempt from the selected vaccination type. Employees who select this check box must select an option from the Exemption Type drop-down list.
Exemption Type If you are exempt from a vaccination, select an exemption type from the drop-down list. Valid options are:
  • Medical
  • Religious
  • Administrative
  • Other
  • Not Applicable
Other Exemption Reason If you select Other as the Exemption Type, enter a brief description in this field.
Note: This field is unlabeled when you use the screen in Form view.
Approval Status If this vaccine type requires approval from your company administrator, this field displays the status of the approval. This field may display the following:
  • Pending
  • Approved
  • Rejected
  • Not Applicable
Reviewer This field displays the employee ID of the company administrator who changed the value in the Approval Status field. The adjacent field displays the selected employee's last and first name.
Review Date This field displays the date when the vaccination data was reviewed by the administrator.
