  • Phrase:Returns all topics that contain the exact phrase in the search criteria. Use quotation marks around the phrase.
    Ex."system administration"
  • And:Returns all topics that contain all the words in the search criteria, in any combination. Insert a plus sign between all words in the search criteria.
    Ex.system + administration
  • Or:Returns all topics with any combination of words in the search criteria. This is the default.

Select New Coverage Level Subtask

Use this subtask to select a new election.

This subtask displays the accidental death and dismemberment insurance plans that you can opt to elect.

Field Description
Select

Select this check box to choose this insurance plan as your new coverage.

Plan

This field displays the insurance plan.

Coverage

This field displays the level of coverage of the plan (for example, "Employee," "Employee + 1," "Family,").

Coverage Amount

This field displays the amount of coverage of the insurance.

<Frequency> (Negative indicates "Cash Back")

This field displays the premium amount of the insurance plan - coverage level, based on the (current) pay frequency ("Monthly," "Semi-Monthly," "Bi-Weekly," or "Weekly").

Plan Information

This field displays the plan information if available. If the plan information is not available, “N/A” displays in the field.

Start Date This field displays the start date of the plan.
Evidence of Insurability

If applicable, this field displays if the coverage option requires Evidence of Insurability (EOI). Valid values are:

  • Not Required
  • Required
EOI Form Location If applicable, this field displays the location where the employee can download or find a copy of the EOI form to fill out.
