Contents of the Employee Personal Info Screen
Use the fields and options to configure the Employee Personal Info Screen.
Employee Personal Information
This box shows your name, Social Security number and date of birth. These fields cannot be changed in this screen. Please contact your HR Administrator if any of the information is incorrect.
Name Preferences
Field | Description |
---|---|
Preferred Name | Enter your preferred name or "nickname". The name you enter here will not be used on official documents, such as paychecks and W-2s. |
Suffix | Enter any suffix that follows your last name, such as Jr., Sr., III, etc. Please be aware that this may be used on your paychecks. |
Demographics
Field | Description |
---|---|
Race/Ethnicity | Click to select the option corresponding to the ethnic group with which you identify. |
Gender | Enter, or click to select, your gender. |
Employment Information
This box shows your job title, hire date, service date, any visa information, your most recent performance review date, and your next performance review date. Your service date is your original hire date if you were rehired. These fields cannot be changed in this screen. Please contact your HR Administrator if any of the information is incorrect.
Visa Information
This box displays your visa information. These fields cannot be changed in this screen. Please contact your HR Administrator if any of the information is incorrect.
Military Status
If you believe you belong to any of the categories of protected veterans, please indicate by checking the appropriate box(es).

Disabled Veteran | Select this checkbox if you are a disabled veteran. A disabled veteran is:

Active Duty Warfare or Campaign Badge Veteran | Select this checkbox if you are an active duty wartime or campaign badge veteran. The U.S. Department of Labor defines “Active Duty Wartime or Campaign Badge Veteran” as a veteran who served on active duty in the U.S. military, ground, naval or air service during a war or in a campaign or expedition for which a campaign badge has been authorized under the laws administered by the Department of Defense. |
Armed Forces Service Medal Veteran | Select this checkbox if you are an Armed Forces Service Medal Veteran. An Armed Forces Service Medal Veteran is a veteran who, while serving on active duty in the U.S. military, ground, naval or air service, participated in a United States military operation for which an Armed Forces service medal was awarded pursuant to Executive Order 12985 (61 FR 1209). |
Recently Separated Veteran | Select this checkbox if you are a recently separated veteran. A "recently separated veteran" means any veteran during the three-year period beginning on the date of such veteran's discharge or release from active duty in the U.S. military, ground, naval, or air service. |
Discharge/Release Date | If applicable, enter or click to select, the date (in MM/DD/YYYY format) on which you were discharged or released from active military duty. |
Protected Veteran (Declined to Self-Identity) | Select this checkbox if you are a veteran who belongs to at least one of the following veterans but has opted not to self-identify a specific category:

Not a Protected Veteran | Select this checkbox if you are not a protected veteran. |
Declined to provide a veteran status | Select this checkbox if you do not want to indicate your veteran status. |
