Form 940 Worksheet Options Tab
The Options tab gives you additional control over the data displayed on the Form 940 Worksheet report.
Reporting Period
Use these options to display the W-2 year and quarter and the last payroll withholding for the posted payroll run.
Field | Description |
---|---|
W-2 Quarter | Select the W-2 quarter for which you want to display quarter-to-date contribution amounts. |
W-2 Year | Select the W-2 year for which you want to display year-to-date contribution amounts. |
Exclusions
Use these options to exclude payroll bonuses and adjustments from the report.
Field | Description |
---|---|
Exclude Bonuses | Select this check box to exclude bonus pay from the wages subject to the tax. |
Exclude Adjustments | Select this check box to exclude payroll adjustments from the wages subject to the tax. |
Employee Sort
Use the Employee Sort options to specify the sort order for the employees included on the report:

Number | Select this button to sort by employee number. |
Last Name | Select this button to sort by employee last name. |

Report Format | Select one of the following formats for displaying employee gross pay, 125/Cafeteria contributions, and total wages subject to tax:

Wage Base | Enter the annual wages that are subject to Federal Unemployment Insurance tax. |
Percentage | Enter the Federal Unemployment Tax percentage to apply to the taxable wages. |
Additional Exempt Payments
Use these options to exclude additional withholdings and contributions from taxable wages.
Field | Description |
---|---|
Withholdings | Select other withholding amounts that you want to exclude from the taxable wages. For example, you might exclude 401(k) contributions. The Add'l Exempt Payments column on the worksheet displays the amount for both the excluded types of withholding and the excluded contributions. |
Contributions | Select employer contributions that you want to exclude from the taxable wages. For example, you might exclude employer matching contributions to 401(k) accounts. The Add'l Exempt Payments column on the worksheet displays the amount for both the excluded types of withholding and the excluded contributions. |
