Columns for the State Form 941 Worksheet

The State Form 941 Worksheet contains a standard set of columns. You do not have the option to select columns for this report.

The header area of the worksheet displays the Form W-2 year and quarter for which you generate the report.

The body of the worksheet displays gross pay, 401(k) contributions, 125/Cafeteria contributions, and the state income taxes paid for each employee.

Columns

Field Description
Employee Number/Name This column displays the employee's identification number and name.
Social Security Number This column displays the person's Social Security number.
Gross Pay This column displays the employee's gross pay for the reporting periods.
401(k) This column displays the 401(k) contributions withheld for the employee for the reporting period.
125/Cafeteria This column displays the 125/Cafeteria contributions withheld for the employee for the reporting periods.
Add'l Exempt Payments If you excluded types of withholding or employer contributions from taxable wages using Additional Exempt Payments on the Options tab, these columns display the total excluded amount for the reporting periods.
State Income Tax This column displays the state income tax amount withheld for the employee for the tax locale, for the reporting periods.
Subject to Tax This column displays the amount of the employee's pay that is taxable for the tax locale for the reporting periods.
Totals This column displays the totals for gross pay, 401(k) contributions, 125/Cafeteria contributions, state income tax, and subject to tax amounts for the employees.

Schedule B Section

This section on the last page of the worksheet displays the daily state tax liabilities for your company, broken out by month and quarter.
