Columns for the Form 941 Worksheet

The Form 941 Worksheet contains a standard set of columns. You do not have the option to select columns for this report.

The header area of the worksheet displays the Form W-2 year and quarter for which you generate the report.

The body of the worksheet displays gross pay, 401(k) contributions, 125/Cafeteria contributions, federal income taxes paid, and the Federal Insurance Contributions Act (FICA) wages for each employee.

Schedule B Section

The Schedule B section on the last page of worksheet displays the daily federal tax liabilities for your company, broken out by month.

Columns

Field Description
Employee Number/Name This column displays the employee's identification number and name.
Social Security Number This column displays the person's Social Security number.
Gross Pay This column displays the employee's gross pay for the reporting periods.
401(k) This column displays the 401(k) contributions withheld for the employee for the reporting period.
125/Cafeteria This column displays the 125/Cafeteria contributions withheld for the employee for the reporting periods.
Add'l Exempt Payments If you excluded types of withholding or employer contributions from taxable wages using Additional Exempt Payments on the Options tab, these columns display the total excluded amount for the reporting periods.
Federal Income Tax This column displays the federal income tax withheld for the employee for the selected period.
Subject to Tax These columns display the total amount subject to tax for the reporting periods, calculated as follows: Gross pay - 125/Cafeteria contributions. The maximum amount subject to tax is the annual wage base that you specified on the Options tab.
Subject to HI This column displays the wages subject to Hospital Insurance (HI).
Subject to OASDI This column displays the wages subject to Old-Age, Survivors and Disability Insurance (OASDI).
