Contents of the Emergency Contacts Screen

Use the fields and options to configure the Emergency Contacts screen.

Field Description
First Name

This field displays the first name of the selected employee.

Last Name

This field displays the last name of the selected employee.

Employee ID

This field displays the code that uniquely identifies the employee.

Title

This field displays the title of the selected employee.

Home Org

This field displays the home organization assigned to the employee.

Status This field displays the employee's status.
Contractor

This check box displays as selected if the employee is a contractor.

Contact 1

These fields display the name and contact information of the employee's primary emergency contact.

Name

This field displays the name of the employee's primary emergency contact.

Relationship

This field displays the primary emergency contact's relationship with the employee.

Phone

This field displays the phone number of the employee's primary emergency contact.

Contact 2

These fields display the name and contact information of the employee's secondary emergency contact.

Field Description
Name

This field displays the name of the employee's secondary emergency contact.

Relationship

This field displays the secondary emergency contact's relationship with the employee.

Phone

This field displays the phone number of the employee's secondary emergency contact.

Change Request Information

Field Description
Status This field displays the status of the change request for this employee's information.
Status Date This field displays the date when the status of the change request was updated.
Notes This field displays the notes entered by either the Human Resources (HR) manager or the employee's manager.
