  • Phrase:Returns all topics that contain the exact phrase in the search criteria. Use quotation marks around the phrase.
    Ex."system administration"
  • And:Returns all topics that contain all the words in the search criteria, in any combination. Insert a plus sign between all words in the search criteria.
    Ex.system + administration
  • Or:Returns all topics with any combination of words in the search criteria. This is the default.

Enter the previously reported box 12 code. If a new box 12 code will added, leave this code as blank because it does not exist in the previously reported box 12 codes and populate its corresponding amount with 0.00.

Previously Reported Amount

Enter the previously reported box 12 amount. If a new box 12 code will added, leave the code as blank because it does not exist in the previously reported box 12 codes and populate this amount with 0.00.

Correct Information Code

Enter the correct box 12 code. If the box 12 code should be removed from the existing box 12 codes, leave it as blank and populate its corresponding amount with 0.00.
