This field displays your current vision plan.

This field displays your current level of coverage (for example, Employee or Family).

This field displays your current deduction amount per pay period for the plan (for example, the premium per month if you are paid monthly).

This column lists all of the vision plans available to you. The plan name may be set up as a hyperlink. If so, click on the hyperlink to see additional information about the plan.

These columns display your vision coverage options (for example, Employee or Family) and associated premiums for different plan/coverage options. Select a plan/coverage combination.

Dependent Information
This table displays if you have one or more dependents set up on the Dependents/Beneficiaries screen.

Covered? |
Select the check box to the left of the dependent's name to indicate that the dependent should be covered under the plan. If the check box is clear, the dependent will not be covered.
The check box is enabled if the selected plan and coverage option require dependents.

This field displays the name of the dependent.

Relationship |
This field displays the relationships of the dependent to you.
