Form 940 Worksheet Columns

The Form 940 Worksheet contains a standard set of columns. You do not have the option to select columns for this report. The header area of the Form 940 Worksheet displays the Form W-2 year and quarter for which you generate the report. The body of the worksheet displays gross pay, 125/Cafeteria contributions, and total wages subject to tax for each employee.

Columns

Column Description
Employee Number/ Name This column displays the employee's identification number and name.
Soc. Sec. No. This column displays the employee's Social Security number.
Gross Pay These columns display the employee's gross pay for the reporting periods.
125/Cafeteria These columns display the 125/Cafeteria contributions withheld for the employee for the reporting periods.
Add'l Exempt Payments If you excluded types of withholding or employer contributions from taxable wages using Additional Exempt Payments on the General tab, these columns display the total excluded amount for the reporting periods.
Subject to Tax These columns display the total amount subject to tax for the reporting periods. Vision calculates these amount as follows: Gross pay - 125/Cafeteria contributions. The maximum amount subject to tax is the annual wage base that you specified on the General tab.
