Medical Benefits

Use the Medical Benefits screen to view and select your medical insurance option(s). You can access this screen through either the Benefits Enrollment or the Life Events/New Hires process.  If you access this screen from the Benefits Enrollment process, it will display your current coverage and your new coverage.  These amounts are broken out by pay frequency with a link to the selected plan information.  You cannot access this screen from the main menu.

When do I use the Medical Benefits screen?

The Medical Benefits screen displays several tables:  your Current Coverage table, which shows your current benefit plan and coverage option information; the Enroll in New Coverage table where you can make a new medical election; the Primary Care Physician Information and Dependent Information tables, on which you can enter your primary care information (when applicable) and select your dependent(s) for coverage (when applicable).  On the Medical Benefits page, you can view, update, save, or navigate away from an existing record, or add a new one.

Current Coverage

View your current medical election (if available) in this table.

Medical Plan

This field displays your current medical election.

Coverage Level

This field displays your current level of coverage (i.e., "Employee," "Employee + 1," "Family," etc.).

<Frequency> Premium

This field displays your current premium amount, based on the (current) pay frequency ("Monthly," "Semi-Monthly," "Bi-Weekly," or "Weekly").

If there is no current dental election for you, the system will display the following message in the Current Coverage Table: "You are currently not covered under a benefit plan.  To enroll, please make a selection below and click the ‘Save’ button."

Enroll in New Coverage

Use this table to make a new medical election.

Medical Plan

This field displays all of the (new) medical election options that are set up in the system. The plan may display as a hyperlink, which you can select to access the Medical Copay table or the plan's website.

<Coverage Level> Options and Medical Plan

These columns display your medical coverage options (i.e., "Employee +Child," "Employee + Family," "Employee + Spouse," "Employee Only") and their rows display the medical premiums for the coverage/plan option combination.  Select the radio button in the column/row that displays your desired coverage/plan option combination.

Primary Care Physician Information

If your selected plan requires a primary care physician, this table will display.

Primary Care Physician Name

Enter your physician's name in this field.

Primary Care Physician #

Enter your physician's identification number here.  

If your selected plan requires a primary care physician and you have not entered the physician’s name or number on the screen, the system will display the following message:  "This plan requires a Primary Care Physician Name and #.  Please enter one for yourself, as well as any dependents assigned to this plan."

Dependent Information

Select a dependent(s) to assign to your plan.  

If a dependent is required for your selected plan and you have not chosen one, the system will display the following message:  "This coverage option requires dependents. Please select a dependent by checking the box next to his/her name below. If no dependents are shown, you may set them up in Dependents/Beneficiaries."

Covered?

Select the checkbox to the left of the row to indicate that the dependent should be covered under the plan.  If the checkbox is unchecked, the dependent will not be covered under the plan.

The checkbox will be enabled if the defaulted/selected plan/coverage option requires dependents.  If the defaulted/selected plan/coverage option does not require dependents, the checkboxes will become unselected (if previously selected) and disabled.

Dependent

This field displays the names of dependents who have been set up in the system.

Relationship

This field displays the relationships of the dependents to you.

Primary Care Physician Name

If the plan that you select requires a primary care physician, the Primary Care Physician Name and Primary Care Physician # columns will display in the Dependent Information table.  Enter the name of your dependent's physician in this field.

Primary Care Physician #

If the plan that you select requires a primary care physician, the Primary Care Physician Name and Primary Care Physician # columns will display in the Dependent Information table.  Enter the identification # of your dependent's physician in this field.

If your selected plan requires a primary care physician and you have not entered one on the screen, the system will display the following message: "This plan requires a Primary Care Physician Name and #. Please enter one for yourself, as well as any dependents assigned to this plan."

Exit Benefits Enrollment

Select this button to return to the splash screen.

Back

Select this button to return to the previous page.

Come Back Later

Select this button to update the database with a placeholder and proceed to the next screen/page.  Then you can revisit the Benefits Enrollment process to complete the election at a later time.

Save

Select this button to update the database with your changes and proceed to the next screen/page.

Medical Plan Copay

Selecting the Medical Plan hyperlink in the Enroll in New Coverage table forwards you to the Medical Plan Copay table (if the information is available in the system).

Coverage Type

This field displays copay information related to the selected plan.

In-Network

This field displays in-network fees for medical expenses as they relate to the Coverage Type description.

Out-of-Network

This field displays out-of-network fees for medical expenses as they relate to the Coverage Type description.
