Select the state abbreviation.

| Employer's State I.D.
|
Enter your enterprise's state identification number. This number appears in Box 16 of the W-2 form. Any associated state wages and income tax withholdings appear in Boxes 17 and 18, respectively. The identification number is assigned to you by your state for the purpose of filing wage and tax reports to state or local government taxing agencies.
