Form 941 Worksheet General Tab

Use the General tab to select report options for the Form 941 Worksheet and to save sets of options for reuse.

Field Description
Report Name You can change the default report name.

Reporting Period

Field Description
W-2 Year Select the Form W-2 year for which you want to generate the report.
W-2 Quarter Select the Form W-2 quarter for which you want to generate the report.

Employee Sort

Specify the sort order for the employees included in the report:

Field Description
Number Sort by employee number.
Last Name Sort by employee last name.

Report Basis

Specify the basis for the report:

Field Description
W2 Quarter Base the report contents on the W-2 quarter.
Current Period Base the report contents on the current period.

Other options

Field Description
Exclude Bonus Select this check box if you do not want the report to include bonus pay.
Exclude Adjustment Select this check box if you do not want the report to include payroll adjustments.

Additional Exempt Payments

Field Description
Withholdings Select other withholding amounts that you want to exclude from the taxable wages. For example, you might exclude 401(k) contributions.
Contributions Select employer contributions that you want to exclude from the taxable wages. For example, you might exclude employer matching contributions to 401(k) accounts.
