Benefits Designate Beneficiaries

Use the Designate Beneficiaries screen to identify, for each benefit plan, your beneficiaries and their allocation percentages.

If you access this screen from the Self-Service menu, you only see information about your current plans. If you access it from the Life Events or Benefits Enrollment process, you see information about both your current plans and newly elected plans.

Current Benefit Plans

Field Description
Plan

This column displays the names of all of your current benefit plans that have associated dependents or beneficiaries.

Beneficiary

This column displays the current beneficiaries for each benefit plan listed.

Relationship

This column displays the relationship of each beneficiary to you.

Type

This column displays the type of beneficiary: Primary, Contingent, or Neither.

Allocation %

This field displays the percentage of the benefit that is allocated to the dependent.

New Benefit Plans

This table displays your newly elected benefit plans and their beneficiaries. Use it to specify the beneficiary type and allocation percentage for each beneficiary, for each plan.

The New Benefit Plans table does not display when you access the Designate Beneficiaries screen from the Self-Service menu.

Field Description
Plan

This column displays the names of all of your new benefit plans that have associated dependents or beneficiaries.

Beneficiary

This column displays the beneficiaries for each benefit plan listed.

Relationship

This column displays the relationship of each beneficiary to you.

Type

Select the type of beneficiary from the drop-down list: Primary, Contingent, or Neither.

Allocation %

Enter the percentage of the benefit that you want to allocate to the dependent.

Selecting Neither from the Type drop-down list disables the Allocation % field.
