Use this application to maintain magnetic media reporting contact information. The Social Security Administration (SSA) requires employers with more than 250 W-2s per employer ID number to report their W-2 data on magnetic media or by electronic filing in the MMREF-1 format.
Enter, or use Lookup to select, the taxable entity ID for which the MMREF record is being created. Taxable entity IDs and information are entered in the Set Up Company Info screen in Costpoint General Ledger. This field can contain up to 10 alphanumeric characters, and the taxable entity must exist in the Set Up Company Info screen.
Use this drop-down box to select one of five options to indicate how this MMREF record was created. Valid system-defined options are "Self-Prepared," "Accounting Firm," "Service Bureau," "Parent Company," or "Other."
Use this group box to enter contact information that will be used by the Social Security Administration and/or other organizations that may have questions or processing problems.
Enter the name of the person or organization that can be contacted regarding this record. You can enter up to 27 alphanumeric characters in this field.
Enter the job title of the contact person for this record. You can enter up to 20 alphanumeric characters in this field.
Use this drop-down box to select the preferred method in which the contact person or organization should be notified. Valid options are "Email/Internet" and "Postal Service."
Enter the email of the contact person or organization. You can enter up to 40 alphanumeric characters. If the Notification Method is "Email/Internet," you must make an entry in this field before saving the record.
Enter the phone number of the contact person or organization. You can enter up to 15 characters in this field.
Enter the phone extension number of the contact person or organization. You can enter up to five characters in this field.
Enter the fax number of the contact person or organization. You can enter up to 10 characters in this field.
