Select the Class Code that applies to an employee.
Valid Class Code options are:
Corporate Officer
Inmate Worker
Job Training Program Act (JTPA) Worker
Partner
Sole Proprietor
Volunteer
Welfare
Independent Contractor
No Class Code
If an employee is designated a Corporate Officer in the Salary Info and History screen, the default entry in the Class Code field is Corporate Officer.
If an employee is assigned a Class Code of Corporate Officer, select the corporate officer ID that applies to the employee.
Valid Corporate Officer ID options are:
President
Vice President
Secretary
Treasurer
Other

Select the coverage type to which an employee is subject. If an employee is subject to both unemployment and workers comp, select Both.
Valid options are:
Unemployment Only
Workers Comp Only

Enter, or use Lookup to select, the geographic code of the employee's primary work site.
Enter, or use Lookup to select, the Alaska occupational code of the employee's occupation.
Enter the five-digit worksite number assigned by Minnesota. If an employee works multiple worksites during the quarter, enter the worksite location at quarter-end.
Select the plan code that applies to this employee's unemployment insurance and disability insurance coverage. Valid Work Plan Code options are:
UI and DI Covered Under State
UI Covered Under State & DI Covered Under Voluntary Plan
DI Covered Under State & Exempt from UI
Covered Under Voluntary Plan for DI Only
Religious Exemption from DI & UI Covered Under State
Covered Under State Plan for UI (Applies to Public Entity Employees Only)

Select the U.S. citizenship status that applies for the employee. This is used to report the U.S. citizenship status of the employee when reporting Virgin Island SUTA information to Ceridian Tax Services (CTS).
