I-9 DATA

U.S. employers can use this screen to track documents they have reviewed for a new hire per the requirements of the U.S. Citizenship and Immigration Services.  Employers can use the I-9 form to establish that the individuals they hire are authorized to work in the United States.

Selections on this screen should reflect the documents provided by the employee to establish both identity and employment eligibility. The I-9 requires either:

U.S. employers can use this screen when an individual that is hired for employment in the United States provides a document or documents that establish both identity and employment eligibility.

Employee Information

Use this group box to identify the employee to whom this record applies and display relevant information stored in the Basic Employee Info screen.

Employee

Enter, or use Lookup to select, the ID for the employee to whom this record applies. This is a required field. The employee's name displays in the unlabeled field to the right.

Address

This non-editable field displays the employee's address from the Address subtask of the Basic Employee Info screen.

Date of Birth

This non-editable field displays the employee's date of birth from the Basic Employee Info screen.

SSN

This non-editable field displays the employee's Social Security Number from the Basic Employee Info screen.

List A

Use these radio buttons to indicate which, if any, of the documents listed have been provided by the new employee to establish both identity and employment eligibility. You can select only one item from this group box, in accordance with U.S. Citizenship and Immigration Services requirements for establishing identity and employment eligibility.

None

Select this radio button if the documents defined in List A do not apply. If you select this radio button, your selections for List B and List C cannot be None.

Expired U. S. Passport

Select this radio button if the employee has provided an expired U.S. passport to establish identity and employment eligibility.

Unexpired U. S. Passport

 Select this radio button if the employee has provided an unexpired U.S. passport to establish identity and employment eligibility.

Unexpired Foreign Passport

Select this radio button if the employee has provided an unexpired foreign passport (with I-551 stamp or attached INS Form I-94 indicating unexpired employment authorization) to establish identity and employment eligibility.

Alien Registration Receipt w/ Photo (I-551)

Select this radio button if the employee has provided an Alien Registration Receipt Card with Photograph (INS Form I-551) to establish identity and employment eligibility.

Unexpired Temporary Resident Card (I-688)

Select this radio button if the employee has provided an unexpired Temporary Resident Card (INS Form I-688) to establish identity and employment eligibility.

Unexpired Employment Authorization Card (I-688A)

Select this radio button if the employee has provided an unexpired Employment Authorization Card (INS Form I-688A) to establish identity and employment eligibility. 

Unexpired INS Employment Authorization w/ Photo (I-688B or I-766)

Select this radio button if the employee has established identity and employment eligibility by providing an unexpired Employment Authorization Document that contains a photograph (INS Form I-766 or I-688B).

List A Documents Acceptable Prior to September 30, 1997

Hint: The following four documents were removed from the list of acceptable List A documents on September 30, 1997.  If the submission date of the I-9 data is earlier than September 30, 1997, these options are acceptable List A documents.

Certificate of U.S. Citizenship

Select this radio button if the employee has provided a Certificate of U.S. Citizenship (INS Form N-560 or N-561) to establish identity and employment eligibility.

Certificate of Naturalization

Select this radio button if the employee has provided a Certificate of Naturalization (INS Form N-550 or N-570) to establish identity and employment eligibility.

Unexpired Reentry Permit

Select this radio button if the employee has provided an unexpired Reentry Permit (INS Form I-327) to establish identity and employment eligibility.

Unexpired Refugee Travel Document

Select this radio button if the employee has provided an unexpired Refugee Travel Document (INS Form I-571) to establish identity and employment eligibility.

Hint: You can select only one of the List A radio buttons. If you select a radio button other than None for List A, you must select the None radio button for List B and List C.

List B

Use these radio buttons to indicate which, if any, of the documents listed have been provided by the new employee to establish identity. You can select only one item from this group box, in accordance with U.S. Citizenship and Immigration Services requirements for establishing identity and employment eligibility.

None

Select this radio button if the documents defined in List B do not apply. If you select this radio button, you must make a selection other than None in List A.

U.S. Driver's License/ID

Select this radio button if the employee has provided a driver's license or ID card that has been issued by a state or outlying possession of the United States and contains a photograph or information such as name, date of birth, sex, height, eye color, or address to establish identity.

Federal/State/Local ID

Select this radio button if the employee has provided an ID card issued by federal, state, or local government agencies or entities, containing a photograph or information such as name, date of birth, sex, height, eye color, or address to establish identity.

School ID w/Photo

Select this radio button if the employee has provided a school ID with photo to establish identity.

Voter's Registration Card

Select this radio button if the employee has provided a voter's registration card to establish identity.

U.S. Military Card or Draft Record

Select this radio button if the employee has provided a U.S. military ID card or draft record to establish identity.

Military Dependent ID

Select this radio button if the employee has provided a military dependent's ID card to establish identity.

U.S. Coast Guard Merchant Mariner Card

Select this radio button if an employee has provided a U.S. Coast Guard Merchant Mariner Card to establish identity.

Native American Tribal Document

Select this radio button if the employee has provided a Native American tribal document to establish identity.

Canadian Driver's License

Select this radio button if the employee has provided a driver's license issued by a Canadian government authority to establish identity.

Hint: You should use the following three radio buttons only for employees who are under age 18 and who are unable to present another document listed above for the List B group box.

School Record/Report Card

Select this radio button if the employee has provided a school record or report card to establish identity.

Clinic/Doctor/Hospital Record

Select this radio button if the employee has provided a clinic, doctor, or hospital record to establish identity.

Day-care/Nursery School Record

Select this radio button if the employee has provided a day-care or nursery school record to establish identity.

Hint: You can use only one of the documents from List B. If you select a radio button other than None for List B, you must also select a document from List C (i.e., List C cannot be None), and you must select the None radio button for List A.

List C

Use these radio buttons to indicate which, if any, of the documents listed have been provided by the new employee to establish employment eligibility. You can select only one item from this group box, in accordance with U.S. Citizenship and Immigration Services requirements for establishing identity and employment eligibility.

None

Select this radio button if the documents defined in List C do not apply. If you select this radio button, then you must make a selection other than None from List A.

U.S. Social Security Card

Select this radio button if an employee has provided an U.S. Social Security card issued by the Social Security Administration (other than a card stating it is not valid for employment) to establish employment eligibility.

Certificate of Birth Abroad

Select this radio button if an employee has provided a Certificate of Birth Abroad issued by the Department of State (Form FS-545 or Form DS-1350) to establish employment eligibility.

Original/Certified Copy of U.S. Birth Certificate

Select this radio button if an employee has provided an original or a certified copy of a birth certificate issued by a state, county, municipal authority, or outlying possession of the United States bearing an official seal to establish employment eligibility.

Native American Tribal Document

Select this radio button if an employee has provided a Native American tribal document to establish employment eligibility.

U.S. Citizen ID Card

Select this radio button if an employee has provided a U.S. Citizen ID Card (INS Form I-197) to establish employment eligibility.

ID Card for Use of Resident Citizen in U.S.

Select this radio button if an employee has provided an ID Card for use of Resident Citizen in the U.S. (INS Form I-179) to establish employment eligibility.

Unexpired INS Employment Authorization Document (other than List A documents)

Select this radio button if an employee has provided an unexpired employment authorization document issued by the INS (other than those shown under List A) to establish employment eligibility.

Hint: You can use only one of the documents from this list. If you select a radio button other than None for List C, you must also select a document from List B (i.e., List B cannot be None), and you must select the None radio button for List A.

Applicable Dates

Use this group box to enter dates that are important to the immigration and naturalization status of the employee.

Date Submitted

Enter the date, in "MM/DD/YYYY" format, on which the I-9 form was signed. This is a required field.

Expiration Date

Enter the date, in "MM/DD/YYYY" format, on which any unexpired forms of identification expire. This is an optional field; however, if you selected any radio buttons for unexpired forms of identification, the system will display a warning message if you try to save the record without entering an Expiration Date.
