Use this screen to set up exit interview forms based on personnel action codes. You can also include or exclude whole sections or line items within each of these sections. Section I will always be printed as shown in this screen.
You can complete this screen at any time after you have established personnel action codes.
Use the fields in this group box to associate an exit interview form with a personnel action.
Enter up to 12 alphanumeric characters to uniquely identify this form. This is a required field.
Enter up to 30 alphanumeric characters to describe the form ID you entered above. This is a required field.
From this drop-down list, select the personnel action that will be associated with this exit interview form. Establish Personnel Action codes in the Personnel Actions screen.
Use this group box to define what will print on the first page of your final exit interview form.
Select the checkboxes to indicate which information you want to print on the exit interview form. This data will come from the Basic Employee Info and the Salary Info and History screens in Costpoint Employee.
Select this checkbox to show the employee's social security number on the exit interview form.
Select this checkbox to show the name of this employee's manager on the exit interview form.
Select this checkbox to show the employee's current hourly rate on the exit interview form.
Select this checkbox to show the name of this employee's supervisor on the exit interview form.
Select this checkbox to show the employee's job title on the exit interview form.
Select this checkbox to show the organization to which this employee is assigned on the exit interview form.
Select this checkbox to show the employee's labor location on the exit interview form.
Select this checkbox to show the employee's current annual rate on the exit interview form.
Enter text that you want to appear above the sign-off signatures.
Use this group box to specify the names of the people who will be directly responsible for signing off on the exit interview form. Enter, or use Lookup to select, the employee IDs of each applicable person. You can select up to 10 employee IDs.
Select this pushbutton to open the Section II - Security and Property subtask, in which you can set up lines for security and property information on the exit interview form.
Select this pushbutton to open the Section III - Compensation and Benefits subtask, in which you can set up lines for compensation and benefits information on the exit interview form.
Select this pushbutton to open the Section IV - Leave subtask, in which you can set up lines for employee leave information on the exit interview form.
Use this pushbutton to open the Section V - Amount Owed subtask, in which you can set up lines for information on amounts owed to the company on the exit interview form.
Use this pushbutton to open pens the Section VI - Comments subtask, in which you can set up lines for employer and employee comments on the exit interview form.
