DEFERRED COMP BENEFICIARY INFO

Use this screen to enter employee deferred compensation beneficiary information to be used with the Employee Deferred Compensation Info screen. You can use this screen to access an employee's deferred compensation beneficiary information in the event of a claim payout.

You can enter beneficiary type, beneficiary names, optional social security number, optional gender, relationship, and percentage assignment.

Use this screen when you first set up deferred compensation benefits or any time the beneficiary information changes.

Employee

Enter, or use Lookup to select, the employee ID. This is a required field. The employee name will default into the adjacent unlabeled field when you enter an employee ID.

Plan

Enter, or use Lookup to select, the Deferred Compensation Plan code for beneficiary assignment. This is a required field. The plan description will default into the adjacent unlabeled field when you enter the plan code.

Plan Date

The plan date will default when you enter Plan code. This is a required field.

Notary Date

Enter the notarized date of the beneficiary information.

Table Window

Type

Enter "P" to denote a Primary beneficiary or "C" to denote a Contingent beneficiary. This is a required field.

Beneficiary Name

Enter the beneficiary name. This is a required field.

SSN

Enter the beneficiary social security number. This is an optional field.

Gender

Enter the gender of the beneficiary. Enter "M" (Male) or "F" (Female). This is an optional field.

Relationship

Enter the employee's relationship to the beneficiary. This is a required field.

Percent

Enter the percentage assigned to the beneficiary. This is a required field.

Note: The total percentage must equal 100% for each primary and contingent beneficiary type. Percentage must be between 1% and 100%.

Changes to this screen update the H_CODA_EMPL_BEN table.
