Use this screen to print a list of employees' life insurance beneficiaries.
You can print or view the Life Insurance Beneficiaries Report at any time.
Use the selections in this group box to control the order in which information prints on the report.
Select this radio button to print information on the report in order by employee name.
Select this radio button to print information on the report in order by employee ID number.
Use this group box to define the effective date and range of employees and benefit plans you want printed on the report.
Enter the effective date of this report in MM/DD/YYYY format. This date is used to determine which beneficiaries to include on the report. The Effective Date is compared to the beneficiary's Effective Date in the Assign Beneficiaries to Benefit Plans screen in Costpoint Employee. This field is required.
Use the fields in this group box to define the range of employees you want printed on this report.
Use the drop-down box to select a range option. Valid options are "All," "One," "Range," "From Beginning" and "To End."
If you selected "One" in the Range Option field, enter, or use Lookup to select, the ID number of the employee for whom you wish to print information on beneficiaries. If you selected "Range" or "To End" in the Range Option field, enter, or use Lookup to select, the lowest ID number in the range of employees you want printed on the report.
If you selected "Range" or "From Beginning" in the Range Option field, enter, or use Lookup to select, the highest ID number in the range of employees you want printed on the report.

If you selected "Range" or "From Beginning" in the Range Option field, enter, or use Lookup to select, the highest code in the range of benefit plans you want printed on the report.
