All of the dependents entered in the Employee Dependents/Beneficiaries screen are automatically enrolled in a medical plan according to the rules of the plan. You can use this screen to link employees, benefit plans, and dependents together to override the default enrollment in medical insurance plans. To assign a dependent to a benefit plan, you must first link the dependent with the employee using the Employee Dependents/Beneficiaries screen. In addition, you must have selected the Dependent or Both radio button in the Type group box in the Employee Dependents/Beneficiaries screen to be able to assign the dependent to a benefit plan in this screen.
You cannot assign dependents to benefit plans until you have set up the Employee Dependents/Beneficiaries screen. After you have set up that screen, you can add to this screen at any time, and enter an unlimited number of records.
Enter, or use Lookup to select, the ID number of the employee whose dependents you want to assign to a benefit plan. Your entry will be validated against the Employee table set up in the Basic Employee Info screen. The name of the employee is displayed in the field to the right of the ID. This is a required field.
Enter, or use Lookup to select, the code for the benefit plan you want to assign to this dependent. Your entry will be validated against the Benefit Plans table. You must have already assigned this benefit plan to this employee in the Employee Benefit Elections screen, and you must have selected the Dependents Eligible checkbox in the Benefit Plan Setup screen in Costpoint Benefits for this benefit plan. The description of the benefit plan is displayed in the field to the right of the code. This is a required field.
The employee's coverage will automatically default into this field. However, the dependent can have a different coverage option if different coverages and rates apply.
If this coverage option uses a rate table, the Dependent Rate per $1,000 will be used to calculate the dependent portion of the premium that will be added to the employee's premium. Otherwise, the system will calculate the dependent's premium and add it to the employee's premium only if the coverage options are different. If you choose to override the default, the coverage option must be valid for this plan (as established in the Coverage Options subtask of the Benefit Plan Setup screen in Costpoint Benefits). The coverage option cannot be "NO CVG." The system will display a warning message if the dependent's coverage option differs from the employee's coverage option, but you can update it if you have entered a valid coverage option.
If the dependents selected by the employee do not match the option selected in the Dep Elig field in the Benefit Plan Setup screen, an error or a warning may display, depending on the Enrollment Rules Edit Method selected in the Benefit Settings screen.
Enter, or use Lookup to select, the ID number of the dependent for whom you want to override medical insurance enrollment. You must have already linked the dependent with this employee in the Employee Dependents/Beneficiaries screen, and you must have selected either Dependent or Both as the Type. In addition, a non-student dependent cannot be over the Dependent Maximum Age as stated in the Benefit Plan Setup screen. A student dependent cannot be over the Student Maximum Age in the Benefit Plan Setup screen. The Dependents Eligible checkbox in the Benefit Plan Setup screen must be selected, and if the Dependent Spouse or Child checkbox is selected, the Relationship, as selected in the Employee Dependents/ Beneficiaries screen, cannot be "Other." This is a required field.
This non-editable field displays the social security number of the dependent as entered on the Employee Dependents/Beneficiaries screen.
The relationship of this dependent to the employee, as entered in the Employee Dependents/Beneficiaries screen, is displayed in this field.
The gender of this dependent, as entered in the Employee Dependents/Beneficiaries table, is displayed in this field.
This non-editable field displays the date of birth of this dependent as it was entered in the Employee Dependents/Beneficiaries screen.
Enter the date, in MM/DD/YYYY format, on which this dependent's coverage begins, if the date differs from the employee's start date. The date must be within the employee's start/end date range as defined in the Employee Benefit Elections screen. If you leave this field blank, the employee's start date will be used. This is an optional field.
Enter the date, in MM/DD/YYYY format, on which this dependent's coverage terminates, if the date differs from the employee's end date. The date must be within the employee's start/end date range as defined in the Employee Benefit Elections screen. If you leave this field blank, the employee's end date will be used. Enter the name of the primary care physician in this field.
Enter the primary care physician's ID number in this field.
Enter the name of an additional physician who is associated with the dependent/benefit plan.
Enter the name of the dentist who is associated with the dependent/benefit plan.
Enter the name of a pharmacy that is associated with the dependent/benefit plan.
Select this pushbutton to populate the table window with the social security number, name, sex, and date of birth all of the dependents for this employee. New rows will be added only for those dependents not already entered in this screen for this employee.
Changes to this screen update the HB_EMPL_MED_DEP table.
